Assign Forms and Track Submissions

Send forms to drivers, start a form for a driver to finish, and review every submission

After you publish a form template, you can send it to specific drivers with a due date, fill it out yourself, start a form and hand it to a driver to finish, and review every submission in one place.

Assign a form to drivers

  1. Go to Forms > Templates.
  2. Click the assign drivers icon (person with a plus) next to the template.
  3. Select the drivers who need to complete it, or use Select All.
  4. Set a Due Date if needed, and click Assign.

Assign vs. Access: Assigning sends the form to specific people who need to complete it. Access, set in the template, controls who can find and open the form on their own.

Fill out a form yourself

  1. Go to the Forms tab and click Add Form.
  2. Select the form and click Start Form.

Start a form and have a driver finish it

Use this when the office needs to fill in some details first, such as pay rate, unit #, or start date, and the driver completes the rest and signs.

  1. Go to the Forms tab, click Add Form, select the form, and click Start Form.
  2. Fill in the fields the office is responsible for.
  3. At the top of the form, use the option to reassign it to the driver.
  4. The driver opens the form in the driver app, fills in their part, signs, and submits.

Ideas for using forms

  • Policy acknowledgments – put the policy text in an Instructions field, then add an Acknowledgement checkbox, the driver's name, a Date, and a Signature.
  • Onboarding packets – build one template per policy, or one template with a section for each policy, and assign it to new drivers with a due date.
  • Agreements with company details – start the form, fill in the company's part, and reassign it to the driver to sign.
  • Contact and information forms – use Text and Phone number fields, or a table for repeated entries like emergency contacts.
  • Quizzes and checklists – use Single select or Multiple choice fields, and turn on Require approvals if someone should review the answers.

Good to know: Forms are built in the form builder and signed in Digital Permit Book. You can't upload an existing PDF and add signature boxes to it, like an e-signature tool. To use an existing document, copy its text into an Instructions field and add the fields and signature you need.

Track submissions

The Forms tab lists every form that has been started, with the Driver/User, Form, Requester, Approver, Submitted at date, Due Date, and Status. Use the search bar and Filters to find a form.

Status shows where each form stands:

  • In Progress – started but not submitted yet.
  • Completed – submitted.
  • Approved or Denied – for forms that require approval.

Click the eye icon to open a form, or the archive icon to move it to the Archived tab.

Get notified: To get an email when a driver submits a form, go to Settings > Notifications (under Profile Settings) and turn on Forms.
